Food and nutrition support

Nutrition services may range from education to meals, groceries, food prescriptions, or cooking supplies for eligible members.

What this can mean

Understand the pathway before expecting a service.

The right nutrition pathway depends on the member’s need, program criteria, and service-specific requirements. A screening alone does not select or authorize a food benefit.

Food supports may be arranged through the applicable network and qualified providers after eligibility and the service pathway are verified.

These are program categories, not an approval list. A Social Care Navigator must verify Medicaid and service-specific requirements.

  1. 3.1

    Nutrition Counseling and Education

    Qualified education may help an eligible member understand food choices and nutrition needs within the applicable program pathway.

  2. 3.2a

    Medically Tailored Meals

    Meals tailored to a qualifying medical need may be available when clinical documentation and program requirements are satisfied.

  3. 3.2b

    Clinically Appropriate Home Delivered Meals

    Home-delivered meals appropriate to a qualifying need may be arranged after eligibility and service criteria are verified.

  4. 3.3

    Medically Tailored or Nutritionally Appropriate Food Prescriptions

    An eligible food-prescription pathway may support foods selected for a verified medical or nutritional need.

  5. 3.4

    Fresh Produce and Non-Perishable Groceries (Pantry Stocking)

    This specialized pantry-stocking pathway is limited to qualifying high-risk children and pregnant or postpartum members after a full eligibility assessment.

  6. 3.5

    Cooking Supplies

    When not available through another program, eligible supplies may include kitchenware, small appliances, a microwave, or a full-size refrigerator, subject to program limits.
